Below is a listing of the registration forms to use for Recreation and Parks (R&P) activities. Please review the information provided under each form in order to insure its correct use. To view these forms so that you can print them you will need to have Adobe Acrobat on your computer.  PAYMENT IN FULL MUST ACCOMPANY REGISTRATION FORMS WHETHER, MAILED, OR HAND DELIVERED. ALL PROGRAMS ARE REGISTERED ON A FIRST COME FIRST SERVE BASIS.

SMC Recreation and Parks Registration Form A

This registration form is to be utilized for all tennis, self-defense, adult leisure, youth leisure, golf, trips, swimming lessons, Therapeutic Recreation classes and programs held at Leonard Hall Recreation Center such as Basketball, Indoor Soccer, Lacrosse, Field Hockey and Roller Hockey. This form is to be used for the Adult Co-Ed Dodgeball TEAMS participating at Carver Recreation Center. The form must be completed and signed by the team captain and turned in with full payment at the time of registration. To be used for all gymnastics programs, located on the current gymnastics schedule. This form is to be used for the Indoor Sports TEAMS participating at Leonard Hall Recreation Center. The form must be completed and signed by each participant and turned in with a full payment at the time of registration. Basketball Registration Form - Instructional - Co-Ed, Youth & High School and Positive Youth Sports Pledge Form F

Registration Form A & Form F are needed for any youth sport registration and must be completely filled out and signed, and can only be hand delivered with payment at location designated in the program information. Driver’s license must be written or printed on all checks.

School Age Care Registration Form G

This form is to be used when registering a child for the Before and After School Child Care, Kindergarten Extended Enrichment Program. This form should be turned in at the designated registration time and location or it can be mailed in (R&P, P.O. This registration form is to be utilized for registration to New Horizons Summer camp AND Camp Inspire. Please indicate on the top of the form which camp you are registering for. This form is to be used when registering for the Fun Camps, Specialty Camps, P.T.O. (Teen) Camps, Jr. Counselor and Sports Camps. Scholarship Application Form J

This form is to be completed and submitted when applying for a scholarship for Recreation and Parks Recreation Division sponsored youth sports (not volunteer youth sports leagues), Before and After Child Care, one week of a summer camp program, Leisure Programs, Recreational Gymnastics Classes and Swimming Lessons. Medication Order Form K

This form is to be completed by your child’s physician if your child requires medication taken during a R&P program. This is a form mandatory form for all individuals taking medication. This form should be turned in at the designated registration time and location or it can be mailed in (R&P, P.O. Box 653, Leonardtown, MD 20650) or brought into the office (Patuxent Building in Leonardtown).

This form is to be completed by your child’s physician. This form is to be used when registering a child for the Before and After School Child Care and the Kindergarten Extended Enrichment Program. This form is to be used when registering a child for the Before and After School Child Care and Pre-Kindergarten Extended Enrichment Program. Parent's Guide (A Consumer Pamphlet) Form P

This form is to be used when registering a child for the Before and After School Child Care, and the Pre-Kindergarten Extended Enrichment Program. This form should be turned in at the designated registration time and location or it can be mailed in (R&P, P.O. Box 653, Leonardtown, MD 20650) or brought into the office (Patuxent Building, in Leonardtown).

 

County Managed Elms Hunting Area Annual Permit Application Form Q

This form is to be used for application for an annual deer bow hunting permit for the 85-acre County Managed Elms Hunting Area at the Elms property.  A liability waiver (below) and separate hunting permit from MD DNR is also needed to hunt on the property.  Applications will be accepted in person weekdays between 8 am and 5 pm, beginning July 1at the R&P main office; applicants may call 301-475-4200 x1800 for an information packet or download forms from the County website at www.co.saint-marys.md.us/recreate and mail in/fax in to: Registrar, Recreation and Parks, P.O. Elms Bow Hunting Liability Waiver Form R

Each Bow Hunter must have a signed “Release and Waiver of Liability” form on file. This form must be completed and submitted to Recreation and Parks with the County Managed Elms Hunting Area Annual Permit Application.
